About the Role
The case manager is responsible for using sound professional judgment and best practices to ensure quality case management services are provided to the service population, especially in crisis situations, while maintaining compliance with established state and federal standards. Monitors an assigned caseload of clients participating in a short-term life management program, serves as an advocate for clients to acquire necessary services, and helps them functionally cope with their environment.
Responsibilities
- Coordinate referrals, service planning, and documentation of services for assigned caseload.
- Interview applicants to determine eligibility for program enrollment based on established guidelines; conduct needs assessments, obtain pertinent information, and establish program goals for eligible clients; refer applicants to other agencies if not appropriate for the program.
- Develop client intake and case plans; instruct clients in completing necessary paperwork, including goals and/or budget forms; complete orientation check-off forms and obtain release of information forms when appropriate; complete social history on new clients weekly.
- Meet regularly with clients to discuss and evaluate their progress; prepare accurate and up-to-date records documenting the same; monitor client behavior, group attendance, medication, and movement; receive incident reports and enforce disciplinary actions for infractions of house guidelines.
- Plan, coordinate, and/or facilitate life management classes to assist clients in obtaining skills that will enable them to functionally cope with their environment; obtain speakers and workshop presenters as needed.
- Serve as a mentor and provide positive examples of proper hygiene, personal appearance, records retention, budget planning, and scheduling.
- Refer clients to appropriate programs or other agencies based on clients’ needs and in accordance with their program plan.
- Complete discharge plans for clients exiting the program; transfer file contents to non-active files.
- Meet all deadlines required by program supervisor and partners.
- Adhere to all federal and state regulatory guidelines and standards applicable to this position.
- Maintain a caseload in accordance with agency policies, procedures, and contract standards.
- Participate in workshops, seminars, education programs, and other activities that promote professional growth and development.
- Maintain a minimum number of weekly contacts with clients.
- Engage and involve clients in the casework process.
- Continually assess ongoing changes in behavior, circumstances, or conditions that may affect child safety.
- Compile, prepare, submit, and maintain accurate records, files, forms, statistics, and additional information in accordance with agency policies, licensing, and/or contract requirements.
- Participate in weekly case staffing with case managers.
- Maintain confidentiality in all areas of the service population and program operations.
- Demonstrate exceptional customer service by placing the child, family, Veteran, or client first to support the mission to "Empower people to build better lives for themselves, their families, and their communities."
Requirements
- 4-year bachelor’s or undergraduate degree from an accredited program/university, preferably in Social Work, Psychology, Criminal Justice, Sociology, or a related field of study.
- Graduate degree from an accredited program/university in a related field (preferred).
- 1+ year of related experience and/or training, including experience in crisis intervention.
- Proficiency in Spanish and English (written and spoken language skills) required.
- Preferred experience in a social or public service environment with case management experience.
- Must maintain regular and acceptable attendance as determined by the employer.
- Valid Driver’s License with a clear record.
- Must have daily use of a vehicle without prior notice.
- Must be available and willing to travel to various locations as business needs dictate.
- Must not pose a direct threat or significant risk of substantial harm to the safety or health of themselves or others.
