Qualifications
- Undergraduate degree from an accredited college or university, or relevant work experience can be used in place of degree.
- 1-3 years of case management experience preferred.
- 2+ years in a customer service-focused environment preferred.
- Experience with veterans is a strong plus.
- Driver’s License must be valid and must be able to successfully pass a criminal background check.
- Must have daily use of a vehicle without prior notice.
Essential Job Responsibilities
- Provide initial intake screenings and wellness assessments to determine client eligibility and coordinate with health and wellness programming.
- Prepare and keep data, files, reports, drafts, and accurate reporting of intake/referral/list activities.
- Collect fiscal data to assist with the completion of monthly, quarterly, and annual reports.
- Advocate for and facilitate client’s access to health and wellness services and community resources and relevant benefits.
- Build a database of community resources. Provide and assist navigation to referrals.
- Manage daily assignments and ensure deadlines are met and work is completed correctly.
- Prepare and maintain confidential case records. Ensure confidentiality and controls access to sensitive information.
- Provide follow-up to ensure clients received the services they were referred to and do not have any follow up needs.
- Develop a network of trusted contacts within organizations in which to refer and coordinate care.
- Consult with service providers and community partners on resource-related issues.
- Conduct initial eligibility screening of clients entering the program and provide general information and community referrals to clients not meeting program eligibility requirements and/or clients not scheduled with a service due to program capacity.
- Complete Intake and Case Management for eligible clients.
- Evaluate individual and family needs. Complete a treatment plan with clients including wellness services and other related needs.
- Cookordinate and monitor services, including comprehensive tracking of client activities in relation to service plan and client’s needs.
- Document detailed case notes, daily; maintain comprehensive client files as per Policies and Procedures.
- Aid the team in conducting service referrals for internal resources.
- Support planning and coordination of the programs in the facility and its activities.
- Responsible for conducting routine psycho-social-emotional needs assessments, completing Person-Centered Recovery Plans, assisting veterans and their families with accessing community resources, providing rehabilitative services, and initiating crisis intervention when needed.
- Promote wellness, community integration, independent living within the community and improved quality of life.
- A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected.
- Obtain, store, and track all client data and coordinate wellness assessments in Salesforce.
- Assist with other administrative duties as assigned.
