Case Manager - Inpatient - La Jolla
About the role
This is a Full-Time position with 10.25-hour shifts on variable days of the week. Position requires working every other full weekend and rotating holidays.
Responsibilities
- Adds addresses payer issues that impact care, develops a transition plan of care from inpatient to post-acute that includes a safe discharge plan.
- Ensures that the patient/family agrees with the transition plan and that the plan is implemented.
Requirements
- RN 2 plus years clinical experience
- 2 years inpatient acute hospital case management experience OR experience in either acute OR non-acute case management.
Qualifications
- Current California RN License
- Certified BLS for Health Care Provider from American Heart Association.
Skills
- BSN highly desired.
- Case Management experience preferred.
- Case Management certification highly desired.
- EPIC experience preferred.
