Case Manager - Inpatient - Encinitas

About the role

This is a Full-Time benefited position with 10.25-hour shifts on variable days of the week including weekend and holiday rotation. Hours are typically 7:30am to 6:15pm. Required to work two full weekends per month. Located at Scripps Memorial Hospital Encinitas.

Responsibilities

Addresses payer issues that impact care, develops a transition plan of care from inpatient to post-acute that includes a safe discharge plan.
Ensures that the patient/family agrees with the transition plan and that the plan is implemented.

Requirements

• RN 2 plus years clinical experience
• 2 years inpatient acute hospital case management experience OR experience in either acute OR non-acute case management.
• CURRENT California RN License
• CURRENT BLS for Health Care Provider from American Heart Association.

Qualifications

• BSN highly desired.
• Case Management experience preferred.
• Case Management certification highly desired.
• EPIC experience preferred.

Pay

Actual pay will be determined based on relevant experience and internal equity within the pay range. A geographical pay differential may be applied for remote employees who reside out of state.
