Case Manager III (Pool/PRN)

What You'll Do

• Facilitate admission intakes, treatment planning, discharge planning, obtaining collateral, clinical interventions, crisis management, assist in the legal process, and initiate personalized care for individuals in crisis and stabilizing from crisis
• Work collaboratively with the interdisciplinary treatment team to coordinate care and discharge planning for individuals, their family members, clinics, local providers, and/or other referral sources
• Carry a caseload
• Conduct thorough, accurate, clinically sound Psychosocial Assessments with individuals in a timely manner
• Review and sign off on Psychosocial Assessments completed by Certified AAC or associate level staff members (if fully licensed)
• Educate and inform individuals of consents for treatment and authorizations for use and disclosures
• Participate regularly in Interdisciplinary Treatment Team meetings
• Collaborate with the Treatment Team to develop and implement individualized person-centered Treatment Plans appropriate for brief intervention; complete signed Treatment Plans and review with each individual and involved parties
• Gather collateral from individuals and involved parties to provide thorough assessment of treatment and discharge needs
• Maintain working knowledge of the Involuntary Treatment Act (ITA) and Evaluation and Treatment (E&T) Involuntary Commitment process; monitor petition process to ensure timeliness and complete forms; attend E&T hearings when subpoenaed
• Actively coordinate care and provide referrals to ensure services are coordinated with facility staff and external providers
• Identify and address gaps in service needs and make appropriate recommendations to meet individuals' needs upon discharge
• Collaborate with the Interdisciplinary Treatment Team to engage, monitor, and communicate with individuals ensuring safety is maintained while meeting their needs
• Participate in redirecting individual behavior while maintaining a safe milieu when necessary
• Document all services and individual activities appropriately
• Provide internal education regarding DBT, trauma-informed care, harm reduction, and service plan documentation to enhance staff ability to effectively address and manage crisis situations
• Provide case management services to promote recovery and community reintegration
• Maintain clinic records, including documentation of activities performed as part of service delivery; obtain signatures from individuals regarding treatment as necessary; document all services and activities in the medical record regarding brief interventions, coordination of care, discharge planning, treatment planning, and other pertinent information
• Prepare clinical reports and individualized service plans utilizing agency electronic clinical documentation software; ensure documentation complies with best clinical practices, contracts, regulatory, and other legal requirements
• Maintain working knowledge of the Washington State behavioral health system and potential funding sources
• Participate in individual and group supervision as required; participate in ongoing education, including in-services, training, and other activities to maintain and improve competency; demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor
• Perform all other duties as assigned

Requirements

• Master's degree in behavioral health, Social Work, Counseling, Psychology, or related discipline
• Unrestricted license as a Licensed Mental Health Professional (LMHP) in Washington State and/or Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or associate license (LICSWA, LMHCA, etc.)
• 3 years of experience in mental health, substance abuse treatment, or relevant work
• Knowledge of the Washington State behavioral health system of care and resources within it
• The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company's exemption process prior to their start date as a condition of employment
• Complete all credentialing requirements within the required time period, including but not limited to obtaining a National Provider Identifier (NPI), Medallion enrollment, Medicare enrollment, and any other necessary credentials as required for the role

Preferred Qualifications

• Experience working with children, youth, families, and adults with behavioral health issues
• Knowledge of and experience with the Washington State involuntary treatment process

Pay

• $10,000 commitment bonus for full-time
• Shift differentials: +$2/hr for Monday-Thursday Nights, +$3/hr for Friday-Sunday Nights, +$2/hr for Saturday-Sunday Days
• Kirkland, Washington Pay Range: $40.50 - $50 USD
