Job Summary
As a Case Management Professional, you will guide patients through complex healthcare journeys to optimize outcomes and well-being. Your responsibilities include assessing patient needs, developing individualized care plans, facilitating seamless transitions, coordinating resources, educating patients, and collaborating with multidisciplinary teams.
Responsibilities
- Assess patient needs and develop individualized care plans
- Facilitate seamless transitions and coordinate resources
- Educate patients and collaborate with multidisciplinary teams
- Ensure continuity of care and prevent readmissions
- Respect and maintain the rights of the patient/family
- Ensure privacy and modesty in the provision of care
- Determine the presence/type of Advanced Directives
- Report suspected cases of abuse/neglect
- Assess physical, psycho-social, cultural, functional, high-risk, and needs of the patient and family
Requirements
- Masters Degree in Mental Health
- Registered Intern with BBS or eligibility
- Basic Life Support - CPR
- One (1) or more years' of experience in an Inpatient Psychiatric Hospital

Serving over 3,000 patients annually, the center offers a full complement of services including behavioral evaluations, a chemical recovery program, and inpatient services. Additionally, St. Joseph’s Behavioral Health Center has been recognized as an LGBTQ+ Healthcare Equality High Performer by the Human Rights Campaign Foundation. It is the only psychiatric and substance abuse hospital in the area.
Pay Range
$45.02 - $51.44 /hour
