Case Manager II - Women & Children's Longterm Program

About the role

The Case Manager II - Women & Children's Longterm Program is responsible for providing comprehensive support services to women and children in long-term care settings. This includes case management, counseling, and coordination of resources to ensure the best outcomes for clients.

Responsibilities

• Conduct assessments and develop individualized service plans for clients
• Counsel clients on various issues related to their needs
• Coordinate with other professionals and community resources to provide holistic care
• Maintain accurate and up-to-date records of client cases
• Participate in regular team meetings to discuss progress and challenges

Requirements

• Bachelor’s degree in Social Work, Counseling, Psychology, or a related field
• At least 2 years of relevant experience in social services or healthcare
• Licenses/certifications: Licensed Social Worker (LSW) or equivalent
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team

Qualifications

• Experience working with women and children in long-term care settings
• Knowledge of trauma-informed care principles
• Proficiency in Microsoft Office Suite

• Excellent organizational and time management skills
• Ability to handle sensitive and confidential information
• Strong problem-solving and decision-making abilities

Pay

$XX.XX per hour

Schedule

Full-time, Monday through Friday, 8:00 AM - 4:30 PM
