CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports they need to thrive. We take a comprehensive approach by offering more than 160 integrated programs in: Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. We reach more than 65,000 individuals and families each year at our 90 locations, including 10,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are living in poverty.
The T Building is a residential building located in the Jamaica section of Queens, developed and operated by Dunn Development Corp. and NY Health and Hospitals. The project has 200 residential dwelling units. CAMBA is the on-site services provider, master leasing 75 apartments for chronically homeless single adults diagnosed with SPMI (seriously and persistently mentally ill). The remaining units are occupied by low-income lottery community residents.
Responsibilities
- Maintain professional relationships with clients and client confidentiality.
- Practice Universal Precautions/Standard Protocol & Procedures.
- Comply with Federal, State, City, and CAMBA security and privacy policies intended to protect individually identifiable health information.
- Review documentation establishing tenants' eligibility for the program and make file copies.
- Create and maintain tenant files.
- Conduct initial intake and assessment of tenants and tenants' families' needs, and periodic re-assessments.
- In collaboration with tenants, prepare and revise independent living plans, including short-term and long-term goals.
- Assist tenants in attaining goals by identifying and locating community resources, and making referrals to appropriate services within and outside CAMBA.
- Help tenants break through barriers to their goals, assist in self-advocacy, and promote self-sufficiency.
- Recommend and implement strategies to encourage tenant participation.
- Monitor tenants' progress via regular telephone contact and/or face-to-face visits, and document progress notes.
- Participate in case conferences.
- Follow up with tenants and referral organizations regarding tenant contact and progress.
- Provide required information for weekly, monthly, quarterly, and annual reports.
- Recommend case closures for tenants who have achieved primary goals, are lost-to-service, or become ineligible.
- Act as tenant liaison/advocate with outside organizations on matters such as education, healthcare, housing, and legal issues.
- Schedule appointments for tenants with referral organizations.
- Escort tenants to appointments (educational, medical, social service, etc.).
- Assist tenants in completing applications for benefits and entitlements, and may process applications on their behalf.
- Follow up with tenants after goal completion to ensure stability.
- Evaluate tenants' living conditions through home visits.
- May prepare marketing materials for the program.
- May prescreen tenants over the telephone for eligibility and schedule intake appointments.
- Input tenant data and progress information into an automated database.
Requirements
- Bachelor’s degree (e.g., B.A., B.S.W.).
- Experience working with the Seriously and Persistently Mentally Ill (SPMI) population and/or equivalent experience.
- Must be fingerprinted and obtain a "Criminal History Check" (CHC) with the Justice Center for the Protection of Vulnerable People with Special Needs.
- Ability to maintain fingerprint clearance throughout employment.
- As a Mandated Reporter, must immediately report suspected incidents/cases of abuse and neglect to the Vulnerable Persons Central Register (VPCR) Hotline and complete a CAMBA Incident Report.
- Flexible hours may occasionally be needed to provide shift coverage until 8 pm.
