Job Duties
- Develops individualized Case Management Plan within one day of enrollment to treatment or into the Recovery Home that includes support services, working with participants and the treatment team to promote overall health and wellbeing.
- Develops Plans that include care coordination, service delivery planning and other strategies that support stability across services and housing transition.
- Facilitates Life Skills groups with consumers on a recurring basis.
- Works as a collaborative team member to ensure all apartment cleanouts, random drug testing, and other collaborative efforts as assigned.
- Walks consumers through enrollment process for allied services.
- Works in collaboration with all FNA treatment units and collaboratively with primary counselor for transition planning to stable housing.
- Works with consumers to understand and address risk factors and connects consumers with resources and support services.
- Affords assistance with Medicaid and health insurance eligibility, linkage to primary and dental care, and linkage to community organizations through the Behavioral Health Community Coalition (BHCC).
- Maintains case load of consumers who reside in our apartments as well as those who need services outside of short-term housing.
- Participates in continuous quality improvement and data-driven decision-making processes to assess program efficacy and drive quality improvement.
- Ensures adherence to data collection requirements and enters accurate and timely data into the Electronic Health Record (EHR).
- Develops billing records.
- Provides culturally relevant and trauma informed service.
- Attends mandatory position training that provides training in risk factors.
Necessary Knowledge, Skills, and Abilities
- Ability to report to work in a timely manner.
- Knowledge of substance abuse and mental health; and appropriate skills in working with consumers with Mental Health/Substance Use disorders.
- Skill in working successfully with the Alaska Native/American Indian population and experience providing culturally specific programming; understanding of and sensitivity to diverse cultures and lifestyles.
- Skill in operating/using computers and using Windows, Word, and Excel with ability to accurately input information into reporting formats.
- Above average communication and people skills with the ability to accurately follow written and verbal instructions.
- Knowledge on how to best assist possible escalated individuals during data collection process.
- Ability to protect confidentiality is essential; HIPAA & 42 CFR compliant.
- Knowledge and ability to interpret/understand billing procedures.
- Skill in managing multiple priorities and tasks concurrently and meeting their deadlines.
- Skill in establishing and maintaining cooperative working relationships with colleagues, supervisors, agencies, and consumers.
Minimum Qualifications (Education & Experience)
- Bachelor’s degree in behavioral health or related field.
- One (1) years’ experience involving case management.
- One (1) years’ experience working with AN/AI population and experience providing culturally specific services.
PREFERRED QUALIFICATIONS (Education & Experience)
- Two (2) years’ experience involving case management.
- Experience working with the LGBTQIA community.
- Master’s degree in behavioral health or related field.
