Full-time, non-exempt position within the Human Services & Health Department.
About the role
This position provides needs assessments, case planning, advocacy, and community-resource development for children and adults with varying abilities and functioning levels. The Case Manager is assigned to a designated Division within the Human Services and Health Department.
Responsibilities
- Engage the client, their family, and natural support system in resolving problems.
- Assess client needs and develop goal-oriented, time-limited service plans.
- Protect children, elderly, and at-risk adults; maintain family units and individual autonomy in the community; prevent out-of-home placement.
- Evaluate prospective clients’ immediate situations, determine appropriate responses, and ensure those responses are delivered.
- Assess and intervene in crisis situations.
- Refer clients to and involve other community resources as needed.
- Collaborate with schools, courts, health-care providers, and other community resources to resolve problems.
- Advise clients when services are unavailable or inappropriate.
- Explain consequences of non-compliance with Court orders to involuntarily-referred clients; monitor service utilization and report adherence to the Court.
- Coordinate access to services, monitor cases for timely and appropriate delivery, review progress, and terminate services when goals are met.
- Teach clients about community resources and how to independently access needed services.
- Advocate for and coordinate services to support clients in achieving treatment objectives without duplication of effort.
- Maintain timely, complete records for all assigned cases; complete required assessments, reports, and documentation.
- Develop and maintain professional knowledge and skills to enhance programs and services.
- Consult with supervisors, interdisciplinary teams, and legal counsel on case-specific matters.
- Facilitate treatment or related group services as indicated.
- Provide public education by speaking to community groups and organizations.
- Perform other duties as assigned.
Requirements
- Must successfully pass caregiver and criminal background checks.
- Case Manager I: Associate’s degree in a related human-services field plus two years of related case-management experience. Equivalent combinations of education and experience may be considered.
- Case Manager II: Bachelor’s degree in a related human-services field (including BSW awaiting licensure, non-BSW with training certificate, or BA in a related field awaiting SAC or related certification).
- Case Manager III: Master’s degree in a related human-services field (no licensure required); OR Bachelor’s degree in a related field with a Social Work license; OR Bachelor’s degree with SAC or related certification; OR licensure by the State of Wisconsin for the employee’s professional affiliation; OR Bachelor’s degree where the position requires billing.
Skills
- Knowledge of laws and regulations governing departmental service delivery.
- Knowledge of community and alternative-care resources.
- Knowledge of applicable State statutes and Wisconsin Administrative Codes (Chapters 34, 35, 36, 48, 51, 55, 63, 75) and Medicaid Waiver Manual (CLTS).
- Working knowledge of mental-health and AODA assessment, diagnosis, and treatment.
- Working knowledge of client rights, confidentiality, standards of practice, and ethics in the mental-health/AODA field.
- Knowledge of human development, behavior, family and group dynamics, and mental, physical, and social dysfunctions.
- Cultural sensitivity and ability to consult on minority-group needs.
- Ability to relate to clients in stressful or crisis situations.
- General knowledge of interviewing, supportive-counseling, and social-casework techniques.
- Knowledge of social, economic, and health problems; available assistance; and rehabilitative services and techniques.
- Knowledge of human-service needs-assessment methodologies.
- Knowledge of legal requirements (privacy, resident rights, confidentiality, non-discrimination).
- Knowledge of human-service program goals, principles, administration, and intended impacts.
- Knowledge of County and Department operating requirements, policies, procedures, and related local, State, and Federal regulations.
- Analytical skill to define problems, evaluate alternatives, and recommend solutions.
- Ability to research, analyze, and make recommendations on detailed information.
- Ability to develop, interpret, and implement regulations, policies, procedures, and related documents.
- Ability to adapt and take control of situations, directing subordinate activities responsibly.

Physical and work environment
- Occasional exertion of up to 25 pounds of force.
- Regularly requires sitting; frequently requires standing, speaking, hearing, and use of hands; occasionally requires walking, stooping, kneeling, crouching, crawling, and reaching.
- Standard vision requirements; vocal communication and hearing at normal spoken-word levels.
- Frequent travel may involve exposure to inclement weather, unsanitary home conditions, communicable illnesses, and unsafe environments.
- May require dealing with hostile, aggressive, abusive, or violent individuals.
- Work performed in office settings, clients’ homes, service-provider facilities, detention centers, treatment and residential facilities, schools, courts, and work sites.
Pay
- Case Manager I: $29.73/hour
- Case Manager II: $31.19/hour
- Case Manager III: $33.32/hour
Schedule
Monday–Friday, 8:00 a.m.–4:30 p.m. No weekends or holidays.
