Case Manager I/II - Employment First

We are seeking a highly motivated, innovative, and enthusiastic staff member to join our Benefits and Community Support (BCS) Division as a Case Manager.

About the role

Case Managers provide professional/technical case management and direct service and assistance to clients within the Employment First and/or Colorado Works Program through the Department of Human Services. This position assists clients in identifying barriers to entering and maintaining employment, and provides assistance and referrals to enable clients to overcome barriers with the goal of entering and maintaining employment.

This position is hybrid and requires at least one day per week in the office. Adequate internet access and space for remote work are required. Larimer County provides a one-time remote work stipend to assist with equipment and office supply needs for remote workspaces.

Schedule

• Monday–Friday, 8 AM – 5 PM

Responsibilities

• Provide individualized case management services to assist clients with the ultimate goal of employment.
• Conduct assessments to identify individual client needs and goals.
• Determine program eligibility and administer services within local, state, or federal guidelines.
• Provide crisis intervention and coaching to assist clients in resolving financial, medical, or other barriers to obtaining and maintaining employment.
• Develop goal plans, make referrals, and coordinate services to meet individual client needs.
• Complete data entry to document goal plans, client progress, and services provided.
• Act as an advocate for the client; coordinate with collateral agencies, families, or service providers; provide follow-up case management to assist clients in resolving problems in receiving quality care and/or services.
• Respond to requests for information and act as a technical and referral source for clients and the public regarding county and community programs.

Requirements

• Ability to think critically and problem-solve and be aware of principles, practices, and objectives as related to human behavior and case management in a human services program.
• Knowledge of the scope and application of laws and regulations pertaining to the assigned and related human services programs.
• Practices and techniques of counseling and interviewing.
• Knowledge of available community resources including employment, medical, social, educational, and financial.
• Responsibility for record-keeping and reporting procedures.
• Maintain sensitive and confidential information.
• Communicate clearly and concisely, both verbally and in writing.
• Possession of a valid Driver’s License by date of hire.
• Complete training and/or certification as mandated by the assigned program.

Qualifications

All candidates will be screened for the following required minimum qualifications. Candidates who do not meet all listed qualifications will be eliminated from the process. Depending on qualifications, this position may be hired as a Case Manager I or II.

Case Manager I

• Equivalent to a Bachelor’s Degree from an accredited college or university (combination of education and/or experience may be substituted on a year-for-year basis).
• Pay: $24.1868/hr – $29.0241/hr

Case Manager II

• Equivalent to a Bachelor’s Degree from an accredited college or university (combination of education and/or experience may be substituted on a year-for-year basis).
• One (1) year of experience in a directly related field or in the performance of similar duties and responsibilities.
• Pay: $26.6055/hr – $31.9266/hr

This position is limited term. Individuals hired into limited-term positions are eligible for County benefits; however, the position is limited in duration dependent upon available funding or business need. The position will be located at one of the following three locations: 1501 Blue Spruce Drive, Fort Collins; 2555 Midpoint Drive, Fort Collins; or 200 Peridot Avenue, Loveland.
