Case Manager I

SAFY’s Greenville, SC division is hiring a full-time Case Manager I to deliver in-home support services for foster parents and children in the Greenville area. This role begins in-office and may transition to hybrid based on performance.

Responsibilities

• Travel daily to provide in-home, community-based services to families and youth on your caseload; most visits will be within 1.5 hours of your home location.
• Complete referral reviews and intake assignments; communicate service availability to all assigned clients.
• Provide instruction, intervention, and assistance—either independently or as part of a treatment team—to preserve the family unit.
• Coordinate with county agencies and attend family sessions to promote successful clinical interventions with foster youth.
• Monitor and coordinate service provisions with school officials, courts, mental-health providers, medical professionals, and other parties involved in the client’s care.
• Develop and ensure completion of assessments—including Individualized Treatment Plans, Psychosocial Diagnostic Assessments (PSDs), and Mental Health Service Plans—within established time frames and in accordance with South Carolina policy and contract regulations.
• Document outcomes of home visits, support sessions, safety assessments, interactions with care parties, and incident reports within 72 hours as required by policy and contract.
• Ensure foster parents, youth, and case manager needs are met during staff transitions.
• Participate in a rotating on-call schedule as required.
• Work nontraditional hours, including some evenings and weekends, to fulfill position duties.

Requirements

• Bachelor’s or Master’s degree in Social Work, Counseling, Psychology, Sociology, or Child & Family Studies.
• Three or more years of experience working with families and children in child welfare or in-home therapies.
• Valid driver license, acceptable driving record, and auto insurance to meet travel requirements; may include transporting youth.
• Current certification in First Aid, CPR, and de-escalation training.
