Case Manager

About the role

Seeking high energy and responsible individual to work collaboratively with men and women who have a Mental Illness to further support wellness, recovery, and independent living skills as well as provide a full range of case management, advocacy and liaison services in our Apartment Treatment Program in Brooklyn.

Responsibilities

• Support individuals with mental illness in achieving wellness and recovery
• Provide case management services
• Advocate for clients' rights and needs
• Liaise with other service providers and community resources

Requirements

• Bachelor’s degree in the Human Services field
• Masters degree preferred

Pay

• Bachelors Level: $55,000
• Masters Level: $65,000
