Case Manager Hennepin and Scott County
About the role
Pinnacle Services is seeking friendly, self-starting professionals to join our Hennepin and Scott County Case Management team. We offer a fun and highly collaborative work environment that centers on providing services of the highest quality to the people we serve, primarily in the six-county metro areas. This role is ideal for someone who enjoys variety, independence, and working with a diverse array of clients.
About two-thirds of the caseload is in Hennepin County and one-third in Scott County. This position requires in-person meetings in both locations, so applicants must be able and willing to travel between the two counties.
Responsibilities
- Assess the areas of the persons served where assistance is needed.
- Offer and coordinate resources for the persons served.
- Monitor service delivery and work with providers to ensure the individual's needs are met.
- Develop service plans.
- Attend regular meetings with persons served and their interdisciplinary teams.
- Case noting client-related work to create billable units.
Case Managers work with persons served on CADI and/or DD waivers who may have a developmental disability, serious and persistent mental illness, chemical dependency issues, a physical disability, or are aging.
Requirements
- Be a graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field.
- OR be a graduate from an accredited four-year college with a major in any field and:
- One year of full-time experience as a social worker/case manager/care coordinator in a public or private social service agency, OR
- One year of experience in education or treatment of persons served with developmental disabilities or related conditions, OR
- At least one course focused on developmental disabilities (official transcript required).
- Possess a valid driver’s license, acceptable driving record, and a vehicle covered with current insurance.
- Successfully clear local and/or state background checks.
Qualifications
Preferred: Possess a LSW or higher form of BOSW Licensure.
Pay
Non-LSW wage: $49,000/year salaried for 40 hours/week. LSW wage: $51,600/year salaried for 40 hours/week.
