About the role

Make a real impact—help someone build independence while building a career you’re proud of. This position requires working in the community and performing home visits.

Responsibilities

  • Work in the community performing home visits.
  • Ensure consumer involvement in the Person-Centered Planning Process (PCP).
  • Facilitate the implementation of Individual Plan of Service.
  • Perform comprehensive, ongoing psychosocial assessments.
  • Participate in legal proceedings including petitioning and testifying in clinical emergencies.
  • Provide community-based services.
  • Facilitate the coordination of consumer intakes, discharges, and transfers.
  • Communicate with all service providers in Treatment Plan.
  • Facilitate hospitalizations, attend hospital meetings, and coordinate with hospital social workers.
  • Coordinate and monitor activities such as job, school, volunteerism, and family caretaking.
  • Provide referral to appropriate community resources.
  • Coordinate all services necessary to maximize consumer recovery and independence for individuals with mental illness.
  • In a residential or clinical setting, provide clinical and program consultation to direct care staff.
  • Ensure case management compliance with agency standards and the practice model.
  • Assist consumers to improve quality of life and maintain the highest possible ability to recover and function within the community.
  • Implement strategies for obtaining services and supports that are goal-oriented and individualized.

Requirements

  • Bachelor’s degree in a human services field from an accredited institution.
  • Valid Michigan Driver’s License.
  • Willingness to work in the community and perform home visits.

Qualifications

Preferred:

  • LLBSW, LBSW, LLMSW, LMSW, LMFT.
  • Prior case management experience in a mental health or rehabilitation setting.
