About the role
Make a real impact—help someone build independence while building a career you’re proud of. This position requires working in the community and performing home visits.
Responsibilities
- Work in the community performing home visits.
- Ensure consumer involvement in the Person-Centered Planning Process (PCP).
- Facilitate the implementation of Individual Plan of Service.
- Perform comprehensive, ongoing psychosocial assessments.
- Participate in legal proceedings including petitioning and testifying in clinical emergencies.
- Provide community-based services.
- Facilitate the coordination of consumer intakes, discharges, and transfers.
- Communicate with all service providers in Treatment Plan.
- Facilitate hospitalizations, attend hospital meetings, and coordinate with hospital social workers.
- Coordinate and monitor activities such as job, school, volunteerism, and family caretaking.
- Provide referral to appropriate community resources.
- Coordinate all services necessary to maximize consumer recovery and independence for individuals with mental illness.
- In a residential or clinical setting, provide clinical and program consultation to direct care staff.
- Ensure case management compliance with agency standards and the practice model.
- Assist consumers to improve quality of life and maintain the highest possible ability to recover and function within the community.
- Implement strategies for obtaining services and supports that are goal-oriented and individualized.
Requirements
- Bachelor’s degree in a human services field from an accredited institution.
- Valid Michigan Driver’s License.
- Willingness to work in the community and perform home visits.
Qualifications
Preferred:
- LLBSW, LBSW, LLMSW, LMSW, LMFT.
- Prior case management experience in a mental health or rehabilitation setting.
