Key Responsibilities
- Scheduling and ensuring that the agency is adequately staffed to provide quality care to clients.
- Responsible for coordinating the client’s visits and communicating with the Talent Acquisition team.
- Responsible for completing on-site home admission assessments and supervisory visits for clients.
- Responsible for developing the Care Plan for coordination of care for all clients.
- Functions as a liaison between the client, family, and the caregiver, actively eliciting feedback from the client.
- Responsible for completing competency assessments and validations for caregivers.
- Maintains records to support and validate caregiver competency according to state rules and regulations.
- Ensures the provision of in-service/staff development programs that contribute to meeting agency objectives and standards and comply with all federal, state, and local regulations.
- Participates in on-Boarding presentations as needed.
- Ensures the Core Values of Patience, Dignity, and Discretion are adhered to at all times.
- Establishes and facilitates effective employee/client relationships.
- Maintain the confidentiality of all pertinent client care information to ensure client’s rights are protected.
- Afford assistance with caregiver assignments when there are shortages (appropriate training needed).
- Responsible for answering the staffing phone to adjust schedule due to unforeseen events.
- Assisting with answering phones as needed.
- Be on-call as needed.
Requirements
- Previous experience in scheduling, managing, and/or healthcare preferred.
- Must have a High School Diploma / GED.
- Willingness to participate in on-call nights and weekends.
- Ability to pass an extensive background check.
- Professional communication skills.
- Previous experience with Axiscare not required but is a plus.
Salary
$60,000 - $70,000 / Year
