Case Manager, Economic Prosperity Services

About the role

The Case Manager will join the Economic Prosperity Services (EPS) team and report directly to the Senior Case Manager. The EPS case management team provides intensive support to residents of the JPNDC affordable housing properties as a part of the RAMWP program. This includes individual case management meetings, coordination with the property management team, and community events at the properties. Additionally, the CM supports the EPS Family Mobility Services team by offering short-term case management assistance to individuals in the greater Boston community. This is a full-time role with a flexible schedule, including some nights and weekends, and frequent site visits. The Case Manager role is ideal for someone passionate about making a positive impact on the community by supporting and stabilizing individuals facing challenging circumstances.

Responsibilities

  • Client Case Management
    • Provide individualized case management from a whole family perspective. Client engagement includes assessment, service planning, implementation, and progress monitoring.
    • Service delivery will follow an individualized service plan that will be created with clients.
    • Work with residents and clients experiencing complex challenges such as rental arrears, income instability, benefits loss, mental and physical health conditions, and/or other barriers to stabilization.
    • Provide support across programs within JPNDC.
    • Coordinate and engage in transition meetings with other departments.
    • Be available to assist with isolated short-term needs for residents in addition to longer-term clients.
    • Work from multiple job sites and meet participants in their homes, if needed.
    • Attend Housing Court when needed as assessed by program supervisor.
  • Resource Knowledge
    • Maintain strong working knowledge of community, city, and state resources and connect clients directly to services including employment, education, legal assistance, housing support, childcare, financial benefits, and emergency resources.
    • Serve as a liaison with external service providers and collaborate on case planning, referrals, and follow-up.
  • Community Outreach
    • Encourage program participation through various outreach methods including but not limited to flyering, presentations, events, and networking.
    • Coordinate community-building events with residents.
    • Develop materials to market events and provide resources to clients.
    • Provide occasional group workshops.
  • Data Management
    • Keep ongoing record of interactions in a timely and professional manner.
    • Track changes and progress through case planning and assessment data.
    • Process referrals through multiple domains.
  • Other Responsibilities
    • Uphold confidentiality, professionalism, trauma-informed, and culturally responsive practice at all times.
    • Perform clerical tasks such as photocopying, scanning, filing, faxing, and printing.
    • Participate in required training and professional development.
    • Perform related duties as assigned by supervisor.

Requirements

  • Bachelor's degree in social sciences and related fields or relevant experience.
  • At least one year of experience providing intensive case management as defined by long-term service delivery, weekly meetings with clients on caseload.
  • Fluent in English and at least one additional language: Spanish or Haitian Creole (written and verbal proficiency required).
  • Experience engaging in assessment and service planning for clients.
  • Experience with documentation of client progress.
  • Experience with trauma-informed care.

Preferred Qualifications

  • Strong crisis management skills.
  • Experience with HUD-funded housing.
  • Navigation of mass housing assistance programs like RAFT.
  • Ability to navigate multiple flights of stairs regularly to access client homes and buildings without elevator access.
  • Ability to lift 20 pounds.

Pay

Annual salary range of $63,000 to $68,000, depending on experience.

Schedule

  • Full-time, salaried position.
  • Five days a week in the office or at project sites.
  • Requires participation in evening meetings.
  • Flexible schedule, including some nights and weekends.
