Responsibilities
- Serve as a Float Case Manager covering program needs when short staffed including advocating for and coordinating client needs and services;
- Ensuring Service Plans, Crisis Plans, and other documentation are completed on time and kept up-to-date, as applicable;
- Providing consistent client contacts, skills training, and care coordination to work towards goals of the client, program, and contract requirements;
- Collaborate with relevant internal and external departments on resident referrals, ensure timely intake into program, and review individualized plans;
- Coordinate with OCRP staff to identify barriers to competency and complete weekly reporting to the courts;
- Collaborate with FHARPS staff to determine housing needs, create plans appropriate to this need, and engage clients in accessing resources to improve housing status;
- Collaborate with FPATH staff to determine goals related to basic needs, attaining benefits, and establishing behavioral health and physical health treatment;
- Coverage of program needs when short staffed, including advocacy for and coordination of client needs and services;
- Ensure all interactions represent the five principles of trauma-informed care, including safety, choice, trustworthiness, collaboration, and empowerment;
- Coordinate access to medications for all clients through trained staff ensuring the seven rights of medication use are followed;
- Complete daily documentation in electronic medical record on client interactions for billable and non-billable activity, medications dispensed where applicable and assigned, and other required documentation;
- Maintain client and staff confidentiality.
Qualifications
- Bachelor's degree in Psychology, Social Work, Counseling, or a related field;
- At least 2 years of experience in behavioral health or related field;
- Experience with trauma-informed care principles;
- Knowledge of community resources and services;
- Ability to work independently and as part of a team;
- Excellent communication and interpersonal skills;
- Proficiency in Microsoft Office applications;
- Valid Washington State Driver’s License and reliable transportation.

Pay
$30.00 - $35.00 per hour based on experience.
Schedule
Monday-Friday, 9:00am-4:00pm (35 hours/week)
