About the role
Empath Trustbridge Hospice is seeking a compassionate and dedicated RN Case Manager to join our team serving patients and families across South Palm Beach County. In this role, you’ll have the unique opportunity to provide comfort, dignity, and support to individuals facing advanced illness—while working alongside a collaborative, mission-driven team.
Responsibilities
- Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
- Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
- Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
- Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
- Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust
Requirements
- Registered Nurse currently licensed in the State of Florida
- Current Basic Life Support Certification
- One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred
- Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
- Basic computer skills and EMR software experience a plus
Qualifications
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license
Skills
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of hospice care principles and practices
