About the role
Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed.
Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team.
Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life.
Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust.
Responsibilities

Requirements
- Registered Nurse currently licensed in the State of Florida
- Current Basic Life Support Certification
- One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred
- Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
- Basic computer skills and EMR software experience a plus
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license
Qualifications

Pay

Schedule
- Monday - Friday 8:30am - 5:00pm
- Every other weekend required
