About the role
We are looking for a Case Manager - Direct Support Professional who demonstrates this mission and wants to work for an organization that makes a difference in the community. This role is responsible for the designated case management tasks of an assigned caseload of up to 16 clients within the I/DD Service Programs.
Responsibilities
- Maintain assigned permanent caseload of up to 16 clients based on programmatic needs; support additional clients as needed and assigned.
- Maintain client case records in the electronic record system (Cx360) per program procedures; review and update annually or as needed.
- Schedule and participate in service planning meetings annually and as needed per licensure requirements.
- Develop and implement individualized strategies coordinating internal and external services to mitigate risks and maximize client progress toward outcomes and community inclusion.
- Coordinate Health Services based on individual client needs and licensure requirements, including scheduling and attending medical appointments and ensuring proper documentation.
- Coordinate Behavioral Intervention Services, including obtaining client/guardian consent and monitoring service delivery.
- Monitor service delivery and client satisfaction with Community Day Services, Behavioral Health Services, and Employment Services.
- Complete and submit referrals for intra-agency services; serve as the point-of-contact to initiate new services and coordinate ongoing service delivery.
- Consult with supervisors, department staff, stakeholders, and providers to secure and coordinate external services and programs.
- Coordinate Specialized/External Transportation Services, including completing required applications and registrations.
- Assist in completing requests for additional funding, services, and supports; collaborate with departments, providers, and regulatory agencies to compile required documentation.
- Demonstrate a strong understanding of client rights and privacy under DHS, HCBS, and CARF standards; monitor services to ensure rights and privacy are protected.
- Assist in developing and implementing strategies for modifications/restrictions of rights as required by licensure; compile supporting documentation for review.
- Review client assessments (e.g., Health Risk Screenings, Nutrition Assessment, Functional Behavioral Assessment) annually or as needed; ensure required timelines are met.
- Collaborate with QIDP and team members to address problems or concerns identified in monthly data analysis and case notes.
- Complete Monthly Progress Reviews to document progress toward outcomes and other updates; consult with QIDP and clients to ensure progress on objectives.
- Obtain signed consents and authorizations for information exchange/services annually or as needed per licensure requirements.
- Perform oversight and monitoring of client financial status, including earned/unearned income, public entitlements, and personal needs allowance.
- Ensure compliance with program guidelines and requirements of Social Security, Medicaid, and Public Aid to maintain benefit eligibility.
- Review, reconcile, and maintain client financial records in collaboration with the Finance Department.
- Provide ongoing financial education, review income, and establish budgets for essential and discretionary expenses at least monthly.
- For clients in agency-controlled properties, complete and review Residency Agreements annually or as needed.
- For clients in affordable/subsidized housing, assist with applications, re-certifications, and lease renewals.
- Complete client case record quality assurance reviews as assigned; provide feedback to primary case managers and supervisors.
- Participate in obtaining input from clients, families, guardians, and stakeholders to improve services through meetings and surveys.
- Compile and enter data on program outcome measures and assist clients in completing satisfaction surveys per CARF standards.
- Conduct at least 2 unannounced and 1 scheduled site visit per week to program locations serving assigned caseload; provide feedback using solutions-focused language.
- Provide direct feedback to staff during visits to ensure compliance with client programming, documentation, safety, rights, and confidentiality standards.
- Report problems or concerns regarding physical environment, clients, or staff to site/program managers and Clinical Director.
- Participate in regular staff meetings and coordinate staff training on client plans and strategies as needed.
- Ensure case management work complies with all agency, state, and federal rules, regulations, and standards.
Schedule
Monday - Friday, 40 hours per week, 1st shift.
