About the Role

The Case Manager will provide direct support to program participants, ensuring engagement with preventative services and removing barriers to accessing care.

Program Context

Southern Illinois Rural Healthcare Services Outreach program (SIRHSO): Focuses on expanding access to preventative health care services in underserved rural areas through the following four national priority areas: Heart Disease, Cancer, Chronic Lower Respiratory Disease (CLRD), and Unintentional Injury. This consortium includes local healthcare providers and community-based partners who are committed to reducing health disparities and improving access to early detection and preventive services in our rural communities.

Behavioral Health - Outpatient Mental Health Programming: Helps individuals build life skills and well-being through behavioral health counseling, psychiatric care, and other care coordination and support services.

Responsibilities

  • Conduct initial participant intake and connect them with appropriate preventative services
  • Manage a caseload of clients, providing ongoing support and case documentation
  • Facilitate community education and outreach campaigns on program focus areas
  • Collaborate with the Registered Dietician to ensure participants have access to nutritional support
  • Monitor participant progress and address barriers to care, such as transportation or financial challenges
  • Track referrals to healthcare providers and measure outcomes for program reporting
  • Develop and maintain relationships with community partners to enhance participant support
  • Assist participants in navigating healthcare systems and accessing additional resources
  • Coordinate follow-ups with participants to ensure continuity of care and engagement in services
  • Provide feedback to the SIRHSO Assistant Director on program challenges and opportunities for improvement
  • Provide critical support services to program participants, ensuring active engagement with grant activities and working to address barriers to successful outcomes
  • Assist with community education and communication campaigns related to specific special focus areas, including heart disease, cancer, chronic respiratory disease, and unintentional injuries

Work Locations

Johnson County Office (JCO), Union County Office - Manion (UCOM), or Alexander County Office (ACO). Location is dependent on office space availability.
