Responsibilities

• Conducting person-centered assessments to understand client needs
• Developing individualized service plans tailored to each person
• Coordinating medical, behavioral, and social supports to help clients achieve their goals
• Connecting individuals and families with community resources
• Monitoring client progress and advocating for necessary services
• Maintaining accurate documentation of all interactions and plans

• Participating in ongoing training to enhance skills and effectiveness
