About the role
Provide comprehensive community-based services to individuals in need.
Responsibilities
- Assist clients with accessing necessary resources and support services.
- Conduct home visits and provide direct care as needed.
- Collaborate with other professionals to develop and implement individualized service plans.
- Document client interactions and progress in accordance with agency policies.
Requirements
- Bachelor’s degree in Social Work, Psychology, or related field.
- At least 2 years of relevant experience in social services or related field.
- Valid driver’s license and access to reliable transportation.
- Ability to work flexible hours including evenings and weekends.
Qualifications
- Strong interpersonal and communication skills.
- Proficient in Microsoft Office Suite.
- Experience with case management software.
Skills
- Excellent organizational and time management skills.
- Ability to handle sensitive and confidential information.
- Knowledge of community resources and services.

Pay
$25.00 per hour, based on experience.
Schedule
Monday through Friday, 8:00 AM - 5:00 PM, with some evening and weekend availability.
