Qualifications/Requirements
- Master’s degree preferred; Bachelor’s degree in social work, psychology, or a related field.
- Experience working in a community-based agency program.
- Proven experience in case management or similar role.
- Strong knowledge of social issues and community resources.
- Excellent interpersonal and communication skills.
- Organized and has good time management skills.
- Proficient in Microsoft Office 365.
- Knowledge of Electronic Health Record Systems (EHR) is a plus.
- Bilingual (Spanish) is a plus.
- Reliable transportation, valid driver's license, and current auto insurance coverage is required.
