Case Manager (CM), Inpatient Rehabilitation
About the role

Responsibilities
- Lead interdisciplinary care coordination and discharge planning
- Identify barriers to progress and help develop solutions with the care team
- Document care activities and complete the Individual Plan of Care (IPoC) per CMS guidelines
- Facilitate weekly team conferences and family meetings
- Communicate patient progress and discharge plans to caregivers
- Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities
- Support additional departmental needs as assigned
Requirements
- Active licensure as an RN, Social Worker (LSW), Respiratory Therapist, PT, OT, or SLP
- 2+ years of case management or social work experience in an inpatient setting (rehab experience strongly preferred)
- Strong communication, organization, and collaboration skills
- Proficiency in Microsoft Office
- Case Management or Rehab Nursing certification (CCM, ACM, RN-BLC, ARN); preferred
Qualifications

Pay

Schedule
This position is full-time and works Monday through Friday from 8:00 AM to 5:00 PM.
