Case Manager (CM), Inpatient Rehabilitation

Responsibilities

• Lead interdisciplinary care coordination and discharge planning
• Identify barriers to progress and help develop solutions with the care team
• Document care activities and complete the Individual Plan of Care (IPoC) per CMS guidelines
• Facilitate weekly team conferences and family meetings
• Communicate patient progress and discharge plans to caregivers
• Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities
• Support additional departmental needs as assigned

Requirements

• Active licensure as an RN, Social Worker (LSW), Respiratory Therapist, PT, OT, or SLP
• 2+ years of case management or social work experience in an inpatient setting (rehab experience strongly preferred)
• Strong communication, organization, and collaboration skills
• Proficiency in Microsoft Office
• Certification in Case Management (CCM), Advanced Case Management (ACM), Registered Nurse (RN)-Board Certified (RN-BC), or Occupational Therapy (ARN) (preferred)

Qualifications

• Active RN license
• 2+ years of case management or social work experience in an inpatient setting
• Strong communication, organization, and collaboration skills
• Proficiency in Microsoft Office
• Certification in Case Management (CCM), Advanced Case Management (ACM), Registered Nurse (RN)-Board Certified (RN-BC), or Occupational Therapy (ARN) (preferred)
