How You'll Contribute

• Complete departmental orientation as well as initial and annual competencies
• Collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge
• Complete documentation in accordance with workflow timelines and content standards, including the Individual Plan of Care (IPoC) per CMS guidelines
• Schedule and conduct family conferences and caregiver communications following team meetings or as needed
• Facilitate weekly care team conferences to review patient progress and adjust treatment plans
• Review the assigned Case Mix Group (CMG) and assist the care team in identifying and documenting treated comorbid conditions; notify the Health Information Management (HIM) team as needed
• Perform other duties as assigned

Qualifications

• Bachelor's degree from an accredited nursing school or Social work program
• Basic Life Support certification is required within 30 days of hire
• ACLS and PALS are required within six months of hire
• One year of experience preferred
• ASLS certification preferred

Pay

$93,600–$104,000 Per year
