Job Summary

The LVN Case Manager works under the direction of an RN following assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client's care. Monitors and transitions care under direction from the RN.

• Required Qualifications:
  • Bachelor's Degree.
  • California Licensed Vocational Nurse upon hire.
  • 2 years of acute hospital experience.
  • One year of supervisory experience.
