Case Manager LPN LVN
Key Responsibilities
- Oversee and manage the delivery of care for residents from pre-admission through post-admission.
- Coincide services to ensure that residents' needs are met in a timely and effective manner.
- Collaborate with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans based on resident needs.
- Maintain monitoring of the progress of residents and ensure appropriate interventions are implemented when necessary.
- Facilitate smooth transitions of care, ensuring all services and resources are in place for each resident.
- Advocate for residents and families, ensuring their needs are met, and provide education as needed.
Requirements
- Successfully graduated from an approved and certified LVN program.
- Current and unencumbered license to practice as an LVN in the state.
- Ability to make quick judgments and work independently.
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care.
Nice to Have
- Two years of experience as an LVN/LPN.
- Active patient or bedside care experience within the past three years.
