Key Responsibilities

  • Oversee and manage the delivery of care for residents from pre-admission through post-admission.
  • Coincide services to ensure that residents' needs are met in a timely and effective manner.
  • Collaborate with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans based on resident needs.
  • Maintain monitoring of the progress of residents and ensure appropriate interventions are implemented when necessary.
  • Facilitate smooth transitions of care, ensuring all services and resources are in place for each resident.
  • Advocate for residents and families, ensuring their needs are met, and provide education as needed.

Requirements

  • Successfully graduated from an approved and certified LVN program.
  • Current and unencumbered license to practice as an LVN in the state.
  • Ability to make quick judgments and work independently.
  • Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care.

Nice to Have

  • Two years of experience as an LVN/LPN.
  • Active patient or bedside care experience within the past three years.
