Phipps Neighborhoods aims to create Pathways to Access, Benefits and a Sustainable Future (PABS) through proactive outreach for a target population of low-income New Yorkers, particularly residents set to experience federal cuts to SNAP, Medicaid, and other public benefits. PABS is designed to help community members navigate this changing landscape by expanding access to benefits, strengthening community-based outreach, and connecting individuals and families to the support they need to remain stable. The initiative is grounded in a data-oriented approach, anti-racist praxis, strong institutional relationship-building, and systems thinking.

Responsibilities

  • Conduct community outreach including educational and listening sessions.
  • Conduct community-based assessments, mapping, and interviews to prioritize health care and social services needs.
  • Help residents connect to available resources by building and maintaining a referral network.
  • Support and motivate community members to achieve health goals by coaching behavior change and identifying strengths and community support systems.
  • Travel extensively to community locations, various agencies, and other outreach destinations for community engagement.
  • Work collaboratively within a team to establish supportive relationships with participants and contribute feedback to strengthen service delivery.
  • Build and maintain positive working relationships with clients, providers, and partner agencies; help reduce cultural and socio-economic barriers between community members and institutions.
  • Perform case management duties.
  • Attend and fully participate in ongoing education sessions to learn and enhance case management skills.
  • Educate community members regarding available community services, health services, patient rights, and provide feedback to be shared.
  • Attend regularly scheduled team meetings.
  • Perform other related tasks as needed, including response to public health emergencies.

Requirements

  • High school diploma or equivalent required; associate or bachelor’s degree in a related field preferred.
  • Experience in community outreach, health education, case management, or social services.
  • Strong communication skills and ability to build trust with residents from diverse cultural and socio-economic backgrounds.
  • Comfort conducting field-based work and traveling locally to community sites and partner agencies.
  • Basic computer skills and ability to enter timely, accurate data into required systems.
  • Commitment to health equity, anti-racist practice, and resident-centered engagement.

Preferred Qualifications

  • Case management and/or other relevant experience.
  • Knowledge of Bronx-based health and human service resources and referral pathways.
  • Experience supporting behavior change, coaching, facilitation, or patient navigation.
  • Bilingual or multilingual skills aligned to community needs.

Pay

$55,000-$60,000 annually commensurate with education and experience.
