ESSENTIAL FUNCTIONS
- Oversee the comprehensive management of case services, ensuring strict adherence to protocols for youth evaluations and assessments
- Maintain meticulous documentation to guarantee compliance with VisionQuest, State, and Federal regulations
- Conduct thorough reviews of all assessments and related documents for precision and correctness prior to submission
- Ensure the timely and accurate submission of all reports and deliverables in accordance with state licensing requirements and VisionQuest's policies and procedures
- Maintain regular contact with various stakeholders involved in the youth's care
- Cookordinate with service providers and community resources to ensure comprehensive care
- Aid with referrals and coordination of any specialty services
- Maintain professional communication with all stakeholders, including family members and guardians
- Monitor and review youth Program Plans and progress
- Understand the individual strengths and needs of each youth on caseload
- Provide supervision and support for youth activities as required
- Maintain security and confidentiality of all youth information including records
- Document all case management actions and communications as required
- Maintain an in-depth understanding of VisionQuest and licensing policies and procedures
- Monitor timeliness and maintain content of all reports, letters, and notifications
SKILLS & ABILITIES
- Excellent interpersonal communication skills with ability to listen actively
- Commitment to professional development and proactive research
- Ability to work independently and as part of a team
- Strong computer skills (Microsoft Office, Google Workspace, Google Drive) with the ability to learn new software
- Excellent writing skills including the ability to produce comprehensive narrative reports
EDUCATION & EXPERIENCE
- Bachelor’s degree in the behavioral sciences, human services or social services fields
- Child welfare and/or case management experience is strongly encouraged
- A minimum of one year of related work experience, preferred
QUALIFICATIONS
- Must be 21 years of age or older
- Flexible schedule (evening, weekends, holidays) to facilitate program services
- Cleared tuberculosis test results
- May be required to pass a drug test
- Cleared background checks from appropriate entities, including Child Abuse and Neglect and FBI criminal background
- Obtain and maintain CPR/First Aid and Crisis Prevention Intervention (CPI) certification
- Must have a valid driver’s license and be eligible to drive to facilitate program services as required
WORKING CONDITIONS/PHYSICAL REQUIREMENTS
- May lift and carry up to 10 pounds frequently, up to 20 pounds occasionally, up to 50 pounds infrequently
- Performs tasks that may involve standing or walking 50% of the day; sitting 50% of the day
- May bend, stoop, reach and grasp as necessary
- Must be able to provide First Aid and CPR
- Must be able to perform physical holds as instructed in Crisis Prevention Intervention training (CPI)
