Case Manager

About the role

The Case Manager is responsible for providing comprehensive case management services to clients, including assessment, service planning, coordination of care, advocacy, and support. The ideal candidate will demonstrate strong organizational skills, clinical understanding, and the ability to work collaboratively with a multidisciplinary team.

Requirements

• Bachelor’s Degree in Social Work, Psychology, Human Services, or a related field or CDCA (required).
• 1–2 years of experience in case management, behavioral health, or social services preferred.
• Strong knowledge of community resources and social service systems.
• Excellent communication, documentation, and organizational skills.
• Ability to manage multiple priorities and maintain professionalism in a fast-paced environment.
• Proficient in Microsoft Office and electronic health record (EHR) systems.
