About the role

TLC Care Center is dedicated to providing compassionate, high-quality care to clients by fostering their well-being and independence. The Case Manager plays a vital role in coordinating personalized care and services for clients.

Key Responsibilities

  • Client Assessment: Conduct thorough evaluations of clients' needs, goals, and resources to develop individualized care plans.
  • Care Coordination: Facilitate access to appropriate services and support, including medical care, housing, education, financial assistance, and mental health resources.
  • Client Advocacy: Serve as an advocate for clients, ensuring their needs are met and empowering them to achieve greater independence.
  • Progress Monitoring: Regularly review and adjust care plans to reflect changes in client circumstances, goals, or needs.
  • Documentation: Maintain accurate and up-to-date records, including case notes, assessments, and care plans, in compliance with organizational and regulatory standards.
  • Collaboration: Work closely with clients, families, service providers, and community organizations to ensure cohesive and comprehensive care.
  • Crisis Intervention: Provide support and intervention during crises, including identifying and mitigating risks to client safety or well-being.
  • Resource Development: Stay informed about available community resources, programs, and services to benefit clients effectively.
  • Training & Development: Participate in ongoing training, supervision, and professional development to enhance knowledge and skills.

Qualifications

  • Bachelor’s degree in social work, psychology, nursing, or a related field (Master’s degree preferred).
  • At least 2 years of experience in case management, social services, or a related role.
  • Knowledge of healthcare systems, social service programs, and community resources.
  • Strong communication, organizational, and problem-solving skills.
  • Proficiency in case management software and Microsoft Office Suite.
  • Compassionate and empathetic demeanor with a commitment to client advocacy.
  • Able to work independently and collaboratively in a fast-paced environment.
  • Certification in Case Management (e.g., CCM) is a plus.
  • Bilingual skills are an asset.

Working Conditions

Office-based with some travel required to meet with clients or attend community meetings. Flexible scheduling to accommodate client needs, including occasional evenings or weekends.
