About the Role
This position involves comprehensive case management for residents, including goal setting, service coordination, and maintaining compliance with Allegheny County Department of Human Services requirements. The role requires regular interaction with residents, community resources, and internal teams to support individuals and families experiencing homelessness.
Responsibilities
- Provide case management, including intake, assessment, and development of Service Plans for residents.
- Maintain and update residents’ case files and progress reports, ensuring confidentiality.
- Coordinate services with community resources, including federal, state, and private agencies.
- Meet regularly with the Director and/or Resident Manager to discuss resident intakes and progress.
- Comply with all DHS-contracted requirements outlined in the Emergency Shelter & Bridge Housing Scope of Service Guidelines.
- Collect and report statistical data as requested by DHS or The Salvation Army Divisional Headquarters (DHQ).
- Process requests for admission to the Family Caring Center and maintain contacts with human/community service agencies.
- Establish and maintain a statistical system for data collection related to residents and referrals.
- Attend required trainings, monthly, and quarterly meetings.
- Assess housing barriers and develop strength-based, solution-focused individualized housing plans.
- Work to reduce households’ average length of stay in emergency shelter and increase exits to permanent housing.
- Meet with residents weekly to update service plans and goals.
- Identify participants’ strengths and barriers, linking them to resources and services.
- Provide mediation and advocacy with landlords and housing providers to secure and maintain housing.
- Create and maintain communication channels between tenants, landlords, referral sources, and collaborating agencies.
- Provide information and referrals to social service agencies and community programs.
- Offer budget counseling and education to assist clients with payment plans and housing stability.
- Educate clients on their rights and responsibilities under residential lease contracts.
- Maintain accurate case notes, monthly outcome reports, and files per contract requirements, including HMIS entry and exit data.
- Collect and report program data, including HMIS reporting and funder-required data.
- Perform other duties as assigned.
Requirements
- Bachelor’s degree in a social service-related field or Associate’s Degree with 7+ years of experience in the Social Service Field.
- Knowledge and experience in human services delivery systems and networking.
- Computer proficiency in Microsoft Word, Excel, PowerPoint, and internet use; ability to back up computer data daily.
- Familiarity with alcohol, drug, and mental health issues, and Act 33 & 34 clearances.
- Understanding of trauma-informed care and sensitivity to the needs of the homeless population.
- Ability to work with individuals and families from diverse social backgrounds and professional levels.
- Must successfully complete The Salvation Army’s Keep Safe required training.
