Case Manager
About the role
LA Family Housing is seeking compassionate and driven Housing Stabilizers to support individuals and families transitioning from homelessness into permanent housing. This role provides intensive case management, housing stabilization services, crisis intervention, and resource coordination for clients facing barriers related to mental health, substance use, chronic medical conditions, income instability, and housing insecurity.
Responsibilities
- Deliver comprehensive case management and housing stabilization services to individuals and families transitioning out of homelessness.
- Conduct client assessments, develop individualized service plans, establish goals, and monitor progress toward housing and life stability.
- Maintain accurate and timely case documentation, case notes, service plans, and database records in compliance with agency and funding requirements.
- Cook up access to critical resources, including housing, healthcare, behavioral health services, substance use treatment, public benefits, employment services, and community support programs.
- Provide crisis intervention, conflict resolution, de-escalation, risk assessments, and safety planning for clients experiencing behavioral health or housing-related crises.
- Advocate on behalf of clients with landlords, property managers, government agencies, and community partners to promote successful housing retention.
- Support clients in developing independent living skills, financial literacy, budgeting practices, and community integration strategies.
- Facilitate client engagement activities, resident events, and wellness-focused programming to foster community connection and stability.
- Participate in multidisciplinary case conferences, team meetings, professional development opportunities, and ongoing training initiatives.
Requirements
- 1+ years of experience in case management, social services, housing programs, behavioral health, community outreach, or related human services fields.
- Experience working with individuals experiencing homelessness, housing insecurity, severe mental illness, substance use disorders, trauma, or chronic health conditions.
- Strong understanding of Housing First, Harm Reduction, Trauma-Informed Care, and Motivational Interviewing principles.
- Demonstrated ability to manage a caseload while balancing documentation, client engagement, and crisis response responsibilities.
- Confident, resilient, and capable of effectively navigating challenging situations with professionalism and empathy.
- Skilled in de-escalation techniques, conflict resolution, crisis management, and client advocacy.
- Highly organized with strong documentation, data entry, and case note management skills.
- Passionate about serving vulnerable populations and committed to advancing housing stability and long-term client success.
Qualifications
- Passport required for travel to other countries.
- Valid driver’s license and reliable transportation.
- Ability to work flexible hours, including evenings and weekends.
- Knowledge of Microsoft Office Suite.

Pay
The pay range for this position is $24.00 - $26.42/hr.
