Position Overview

SSM Health Rehabilitation Hospital is a joint venture between Select Medical and SSM Health. This inpatient rehabilitation hospital is dedicated to providing exceptional and compassionate care for individuals with various medical conditions.

About the Role

The Case Manager coordinates health care decisions, ensures treatment plans improve quality and outcomes, and manages care across the continuum. They also promote cost-effective care within the allotted time frame and implement safe and appropriate discharge plans. The Case Manager assesses psychosocial needs and provides intervention during the discharge planning process.

Requirements

• Current Licensure per state guidelines in clinical or related discipline (RN preferred)
• A Bachelor's or Master's in a health or human services discipline
• Previous experience in Case Management and Discharge Planning preferred
• CCM Certification preferred

Pay

$77,000 - $105,000 based on years of experience

Schedule

Full-Time Monday through Friday 8 hour days
