Job Summary

SSM Health Rehabilitation Hospital, a joint venture with Select Medical, is seeking a Case Manager (SW/RN) for a per diem position in Oklahoma City, OK. The position requires a strong background in case management/discharge planning, particularly in behavioral health settings.

About the Role

The Case Manager plays a crucial role in coordinating health care decisions, ensuring treatment plans that enhance quality and outcomes, and managing care across various healthcare settings. They also focus on promoting cost-effective care and implementing safe, appropriate discharge plans.

• Payer/Referral Management

Requirements

• Current Licensure per state guidelines in clinical or related discipline
• Current RN license preferred
• Current SW license preferred
• Bachelor's or Master's degree in a health or human services discipline
• Previous experience in Case Management and Discharge Planning preferred
• CCM Certification preferred

Qualifications

• Experience in case management/discharge planning, particularly in behavioral health settings
• Strong communication and problem-solving skills
• Ability to manage multiple tasks and prioritize effectively
• Knowledge of healthcare policies and regulations
