Case Manager
Starting salary range: $52,797 - $54,465 plus a $130/month office benefit package (parking/remote expenses), subject to change.
About the role
The Case Manager provides person-centered services to promote recovery and community integration of adults with serious, persistent mental illnesses and co-occurring substance use disorders within Ramsey County. Our goal is to help clients obtain and maintain an optimum quality of life that aligns with their needs and preferences. Case Managers assist clients in accessing needed medical, social, educational, vocational, and other necessary services related to their mental health needs.
This is a full-time, hybrid position. Some work is done in the community with clients, and other work can be performed remotely from home.
Responsibilities
- Develop and implement treatment plans based on diagnostic and functional assessments, approved by the clinical supervisor.
- Ensure compliance with Minnesota Rule Chapter 9520 alongside other team members.
- Provide or ensure access to eligible medical, vocational, social, educational, financial, legal, housing, and chemical dependency services.
- Develop an Individual Community Support Plan (ICSP) for each client every 6 months.
- Schedule face-to-face visits as dictated by the ICSP.
- Make culturally sensitive referrals and assist clients in accessing needed services.
- Participate in the commitment process as mandated, including providing testimony in probate court, writing required court reports, monitoring stayed warrants, provisional discharge agreements, and assisting in the pre-petition screening process.
- File legally required reports with Vulnerable Adults, the Ombudsman, and Child Protection.
- Provide services to maintain and enhance each client's ability to live as independently as possible.
- Develop individualized emergency support plans coordinated with community emergency services.
- Assist clients with housing issues and interface with HRA, PHA staff, and landlords.
- Attend scheduled quarterly reviews and treatment planning meetings.
- Collaborate with, support, and educate family members and others involved in the client's treatment and welfare when warranted.
- Educate clients about the symptoms, course, and treatment of their illness.
- Provide team and agency coverage of client needs.
- Develop appropriate therapeutic relationships with clients.
- Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
- Participate in ongoing professional growth and development, maintain appropriate professional behavior, and engage in supervision.
- Transport clients in the community to various appointments.
Requirements
- Bachelor’s degree in behavioral sciences or a related field (if no degree, 30 semester credit hours in behavioral health sciences) and 2,000 hours of supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
- OR a Master’s or other graduate degree in behavioral health sciences or a related field.
- OR if no degree, 4,000 hours of supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
- Must have a car, auto insurance, and a valid driver’s license.
Schedule
Monday–Friday, 8:00 AM–4:00 PM or 9:00 AM–5:00 PM, 40 hours per week.
