🚨 NOW HIRING: CASE MANAGER - NYC AGING SERVICES 🚨📍 Location: New York City, NY 💼 Position: Case Manager - Older Adult Services 📅 Employment Type: Temporary Assignment 🏠 Work Setting: Office-Based & Community/Home VisitsWe are seeking a compassionate and dedicated Case Manager to support older adults and their families throughout New York City!This is an excellent opportunity for professionals with a background in Social Work, Human Services, Case Management, or Community Outreach who are passionate about improving the quality of life for NYC's aging population.KEY RESPONSIBILITIES:Conduct in-home and office-based assessments of older adults' social, emotional, and support needs.Develop and implement individualized care plans.Coordinate essential services, including transportation, home care, Meals on Wheels, and community resources.Assist clients with Medicare, Medicaid, public assistance, and other benefits and entitlements.Provide counseling, guidance, advocacy, and crisis intervention.Collaborate with families, social workers, healthcare professionals, and community agencies.Monitor client progress, reassess needs, and update care plans.Maintain accurate case documentation, reports, and service records.REQUIREMENTS:Bachelor's degree in Social Work (BSW), Human Services, or a related field; BA/BS or equivalent accepted.Strong communication, organizational, and documentation skills.Computer proficiency and ability to learn case management software.Ability to travel throughout NYC using public transportation and conduct home visits.Experience working with older adults or vulnerable populations is highly desirable.Bilingual candidates speaking Spanish, Russian, Mandarin, Cantonese, or Patois are encouraged to apply!🌟 Make a meaningful difference in the lives of older adults while gaining valuable experience in community-based case management!📩 Interested?
