Case Manager

About the role

This is a 13-week contract position requiring 5+ years of case management experience, with at least 2+ years of recent acute inpatient case management experience.

Responsibilities

• Manage patient care plans
• Collaborate with interdisciplinary teams
• Facilitate discharge planning

Requirements

• Bachelor of Science in Nursing (BSN) or Master of Social Work (MSW)
• Current Michigan Registered Nurse (RN) license or Licensed Master Social Worker (LMSW)
• Experience in discharge planning
