About the role

As a member of our team at Lifeworks, a nonprofit human service agency, you will support adults with developmental and intellectual disabilities, providing advocacy, empowerment, and opportunity. We are seeking an enthusiastic Case Manager who is committed to enhancing the lives of those we serve through creative opportunities and services.

Schedule

Monday–Thursday 8:00 AM–4:00 PM, Friday 7:30 AM–3:30 PM

Pay

$48,160 per year

Responsibilities

  • Promote optimal health for individuals through routine, preventive, and emergency health care.
  • Ensure safe, secure work environments and Life Enhancement activities where individuals are free from harm.
  • Provide supervision, training, and coaching across all settings to increase individuals’ self-sufficiency and independence, meeting the needs of employers, volunteer sites, families, and individuals supported.
  • Support employers as necessary to ensure quality and consistency of work performed by individuals.
  • Provide education, training, counseling, guidance, and physical assistance to achieve individual goals in employment, education, social growth, health and wellness, communication skills, community mobility, and volunteerism.
  • Develop, write, and present annual Individual Support Plans (ISPs) and subsequent support paperwork in consultation with your supervisor.
  • Advocate for and ensure ISP plans are implemented to achieve individuals’ goals through employment and the Life Enhancement Program.
  • Perform case management duties, including documentation of training and activities, maintenance of confidential files, and submission of required paperwork according to agency procedures.
  • Attend regular meetings as necessary.
  • Be flexible regarding changes in work schedule and tasks as assigned by your supervisor to meet the needs of individuals supported.
  • Maintain regular communication with individuals, ISP team members, and significant others (e.g., family, guardians, advocates) to ensure consistency of programming and support.
  • Act as a liaison between Lifeworks, employers, volunteer sites, and community-supported sites to ensure individuals meet all expectations.
  • Maintain confidentiality and professional demeanor with all individuals and staff members.
  • Provide transportation to and from various community sites using Lifeworks or personal vehicles; staff are reimbursed at the agency mileage rate for personal vehicle use.
  • Demonstrate good computer skills and the ability to learn agency-specific computer programs for documentation.
  • Perform other duties and assignments as needed based on company requirements.

Requirements

  • Experience working with individuals with intellectual and developmental disabilities (ID/DD).
  • A Bachelor’s degree, or an Associate degree in a related field with two years of experience, or a high school diploma with five years of related work experience in Human Services.
  • Spanish-speaking ability is a plus.
  • Must pass fingerprint background check, CORI, DPPC, and driving background checks.
  • Valid Driver’s License required.
  • Valid MAP certification is preferred.
  • Excellent verbal and written communication skills.
