Case Manager

About the role

As a Case Manager for Anchor House, you will provide case management functions and duties for youth and families receiving Idaho Youth Ranch services. This collaborative position involves working with the Therapist, Program Manager, client family, and referral sources to meet the comprehensive needs of the client.

Responsibilities

• Provide case management to clients with a behavioral health diagnosis who are unable to navigate or coordinate the service system independently.
• Assess, plan, link, coordinate, and monitor options and services that address a client’s needs.
• Help clients learn, gain, and maintain access to services and providers.
• Provide skill-building services specific to goals identified in the individualized treatment plan to the point where the client may be safely, efficiently, and effectively treated in the least restrictive service level.

Requirements

• Bachelor’s Degree in social work, psychology, counseling, or a closely related field as identified by inclusion of significant coursework in psychology, child development, and related studies.
• 1 year of experience in case management, preferably serving Medicaid clients.
• Ability to complete annual certification of CANS.
• Passion for working with youth and families.
• This position is located in Idaho, and candidates must be within a reasonable distance to our location.
