Successful candidates will have a once-in-a-career opportunity to be part of the opening of our new Residential Center for Healing and Resilience, a 64-bed psychiatric residential treatment facility (PRTF) for youth, at our beautiful 258-acre Hands of Promise Campus in Idaho’s Treasure Valley.

As a Case Manager, you will provide case management functions and duties for youth and families receiving IYR services. This is a collaborative position meaning you will work with the clinical and medical team at the RCHR, client family, and referral sources to meet the comprehensive needs of the client.

Requirements

• Social Work Licensure or Bachelor’s degree in behavioral science or related field with 1 year experience with children in a social service setting or 5 years experience with children in a social service setting
• Ability to complete annual certification of CANS
• Demonstrate knowledge of Medicaid’s Youth Empowerment Services Program
• Demonstrate a working understanding of HIPAA and Medicaid guidelines and ability to maintain youth confidentiality and privacy

Responsibilities

• Provide case management for youth receiving inpatient residential care
• Assess, plan, link, coordinate, and monitor options and services that address a youth and family’s needs
• Help clients learn, gain, and maintain access to services and providers
• Provide skill-building services specific to goals identified in the individualized treatment plan to the point where the client may be safely, efficiently, and effectively treated in the least restrictive service level
