Case Manager

About the role
The Strobel Center operates 24/7, providing accommodation and support to homeless men and women with disabilities. The Case Manager conducts assessments to determine client needs and plans for obtaining services, offering direct assistance, care coordination, and linkage to appropriate services while developing support plans for residents. This role reports to and works closely with the Nashville Director to move clients toward greater stability.
Responsibilities
- Provide guidance and support for project residents, ensuring their practical needs (e.g., food, laundry, clean bedding) are met in conjunction with volunteers.
- Maintain a safe, secure environment and high operational standards within the project.
- Assess resident needs and refer them to appropriate agencies and social service programs.
- Develop and update support plans for residents (at least weekly), recording work, advocating for clients, organizing files, and reporting progress to the supervisor and project team.
- Serve as a liaison with Metro Nashville OHS, emergency services, and other public/private social services agencies, particularly regarding housing, health, and benefits.
- Build professional relationships with residents and assist them with difficult or problematic situations sensitively.
- Assist with reception and administrative duties as scheduled.
- Collaborate with staff during shifts and make decisions consistent with project team agreements.
- Transition pertinent information to other staff at shift changes and support volunteers.
- Assist in preparing reports, statistics, and other documents for the Nashville Director.
- Conduct data entry and maintain a computerized monitoring system.
- Provide facility tours for visitors and volunteers.
- Ensure residents complete all required records and forms.
- Maintain records necessary for smooth project operations.
- Build working relationships with other projects and agencies.
- Stay informed about local and national events, trends, and issues impacting Depaul USA’s work with the homeless.
- Adhere to quality assurance policies and procedures.
- Participate in internal/external meetings, training, conferences, and supervision as required.
- Identify developmental and training needs during annual evaluations.
- Ensure adherence to Depaul USA policies, including Health and Safety, Safeguarding, and Confidentiality.
- Contribute to implementing Depaul USA’s Equal Opportunities Policy.
Requirements
- Associate degree in social work, counseling, psychology, or a closely related field.
- At least two years of experience working with populations affected by behavioral health diagnoses and homelessness.
- 1+ years of experience in addictions counseling.
- Exceptional communication, multi-tasking, conflict management, and interpersonal skills.
- Fluency in Spanish is highly desired.
- Strong computer skills, including proficiency in Microsoft Office programs.
- Ability to organize tasks, events, and people effectively and delegate when appropriate.
- Strong decision-making skills and ability to exercise independent judgment.
- Valid driver’s license and compliance with Depaul USA’s Motor Vehicle Policy; occasional use of a personal vehicle may be necessary.
Physical Demands
- Sitting, standing, climbing stairs, walking, driving, carrying (up to 20 lbs), and lifting (up to 20 lbs).
- Operating a computer keyboard and making/receiving telephone calls.
- Exposure to outside environmental conditions.
Schedule
- Primarily in-person at the Strobel Center in Nashville, TN.
- Monday–Friday, 7:00 AM–4:00 PM CST.
- Must be available to work weekends and/or evenings as needed.
Pay
$50,000–$55,000 per year, paid bi-weekly.
