Responsibilities

  • Collections information about the patient’s situation and functioning to identify individual needs.
  • Assessment of that information to develop a comprehensive case management plan.
  • Plans the specific objectives, goals and actions designed to meet the patient’s needs.
  • This plan should be action-oriented and time-specific.
  • Establishes, communicates, and facilitates initial program issues and goals with appropriate designated staff.
  • Communicates with external stakeholders and families to ensure coordination of services.
  • Implements the specific case management activities and/or interventions that will lead to accomplishing the plan goals.
  • Evaluates, at appropriate intervals, the plan’s effectiveness in reaching the desired outcomes and goals.

Qualifications

  • Passion for Care: A deep understanding of the requirements for providing the type of care and supervision needed, including the ability to communicate effectively with them.
  • Leadership Skills: The ability to recruit, employ, train, and evaluate qualified staff, and to make tough decisions when necessary.
  • Experience And Education: Educational Background: Bachelor’s degree in a related field required. Master’s or licensure/credential preferred but experience can be used to offset this level of education/licensing.
  • Work Experience: Minimum of 3-5 years’ experience in rehabilitation or related field.
  • Specialized Knowledge: CCM preferred. Successful completion of CCM required upon meeting eligibility guidelines.
  • Demonstrate a capacity for and interest to absorb a global knowledge of medical and clinical rehabilitation aspects relating to traumatic brain injury.
  • Experience in which supervisory abilities have been demonstrated.
