About the role

Inspire of McClelland provides comfort of home to independent elders, offering leisurely access to delicious & nutritious meals, wellness & social activities, assistance with tasks of daily living, and medication assistance. We are seeking a positive, dependable, and caring Case Manager to join our team and meet the growing demand for quality care in our community.

Responsibilities

  • Make daily rounds to assess and monitor the general condition of all residents.
  • Provide general staff supervision to ensure the provision of quality care to residents.
  • Assist in staff orientation and staff scheduling.
  • Participate in staff scheduled meetings as necessary.
  • Work with Medication Staff to check medication inventory and oversee the safe storage and disposal of medications.
  • Liaise for residents between physicians, community health agencies, social agencies, family members, and other significant individuals.
  • Collaborate with residents to resolve concerns and issues.
  • Document all significant incidents pertaining to resident condition.
  • Review appointment forms for doctor visits when residents return from the doctor or clinic.
  • Adjust medication changes and orders in resident charts and on the medication administration record.
  • Assist with residents' charts and orders when the House Doctor and House Psychiatrist visit.
  • Provide accurate and up-to-date documentation of residents’ care management needs, including problems identified and interventions taken.
  • Document and submit all incidents pertaining to resident conditions, staff, and visitors.
  • Visit hospitals, private homes, and long-term care facilities to assess and evaluate prospective residents.
  • Conduct quality assurance by evaluating all current interventions to ensure care measures are closely monitored and documented.

Requirements

  • Ability to communicate in a dignified, respectful manner.
  • Relates well to residents, visitors, and other staff members.
  • Emotional and physical stamina to assist residents in their daily living skills.
  • Interest and participation in opportunities for personal academic growth.
  • Ability to practice self-control in speech, listening, body, and facial mannerisms.
  • Technologically inclined with a working knowledge of the Microsoft Office suite.
  • Possess physical endurance for frequent activity.

Qualifications

  • Must be at least 18 years of age.
  • An Associate Degree and three years of full-time experience in the provision of services to a dependent adult population OR a Bachelor's Degree and one year of full-time experience in the provision of services to a dependent adult population.
  • Prefer previous experience, either professional, personal, or volunteer, with the elderly population.
  • Must be proficient in Microsoft Office Suite.

Pay

$25–28 per hour, plus paid time off (PTO).
