Job Details

Full‑time, non‑exempt position reporting to the Case Management Supervisor. Pay Grade: 20B.

Summary

Assist adults with disabilities and older persons to assess their needs, authorize and obtain in‑home and community‑based services to maintain independence, avoid institutional settings, facilitate timely return home after short stays, and remain at home with support despite functional impairments. Develop and administer a service plan that maximizes independent living while addressing acute issues.

Essential Functions

• Perform comprehensive functional assessments and reassessments for clients.
• Develop appropriate plans of care to achieve identified outcome objectives.
• Implement short‑term and long‑term intensive service plans, including authorization of specific in‑home and community services.
• Resolve client‑directed concerns with service delivery quality.
• Coordinate with providers of client services.
• Plan termination and conduct follow‑up.
• Utilize in‑house nursing staff for case consultation, service plan development, and provider oversight.
• Maintain manual and computerized client records.
• Coordinate services with other community providers, including mental health and disability service organizations.
• Monitor the TXIX CM phone line, listening, responding, and triaging situations as assigned.

Working Conditions and Physical Effort

Assessments are performed in varied residential environments, requiring travel by automobile and exposure to changing weather conditions. A portion of work occurs in a typical interior/office setting. The role regularly involves talking, hearing, standing, walking, sitting, using hands, and reaching. Occasionally lifts or moves up to 20 pounds. Vision requirements include close focus. Reasonable accommodations are available.

Requirements

• Master’s degree in behavioral or health science and one year paid on‑the‑job social service experience, or Bachelor’s degree in behavioral or health sciences and two years of paid on‑the‑job social service experience, or Bachelor’s degree and four years of paid on‑the‑job social service experience, or a relevant combination of education and experience.
• Valid driver’s license, state‑required vehicle insurance, and reliable transportation.

Knowledge, Skills, and Abilities

• Knowledge of local in‑home and community options and resources for the elderly and adults with disabilities and their caregivers.
• Direct functional assessment, service planning, and implementation experience.
• Ability to function in a multilingual, multicultural environment, including use of interpreters.
• Ability to travel to client homes and community agencies, including locations that may not be ADA accessible.
• Ability to maintain paper and electronic records and report accordingly.
• Ability to establish and maintain effective working relationships with clients, families, caregivers, service providers, medical personnel, and agency staff.
• Ability to work independently in the field and under pressure with short timelines.
• Strong verbal communication in English in one‑on‑one, group, computer, and telephone settings.
• Proficient written communication with proper English grammar, punctuation, and organization.
• Computer and software skills; ability to operate general office equipment and work at a desk using phone and computer up to eight hours daily.

Schedule

Pay

Salary typically starts at $66,096–$91,128.
