Case Manager

Job Summary

A mission-driven nonprofit organization in Los Angeles is seeking a Case Manager for a temp-to-hire opportunity supporting veterans as they work toward stability, independence, housing security, and access to community resources. This Case Manager role is a great fit for someone with 1 to 2 years of experience in social services, administrative support, client services, or a related field.

Key Responsibilities

• Aid with client intake, eligibility documentation, file setup, and ongoing case management documentation.
• Support veterans by assessing needs related to housing, employment, education, financial stability, legal concerns, family support, medical care, and behavioral health resources.
• Cook up services and referrals with community partners, healthcare providers, housing resources, and internal program teams.
• Help develop, update, and document Individual Action Plans, progress notes, discharge summaries, and service referrals.
• Maintain accurate, confidential client records in compliance with HIPAA standards and organizational requirements.
• Participate in team meetings, trainings, quality management activities, data entry, reporting, and client support activities.

Compensation And Benefits

• Salary Range: $48,000 to $52,000 per year.
• Job Type: Temp-to-Hire.
• Schedule: 8:00 AM to 5:00 PM or 8:30 AM to 5:00 PM.
• Location: Los Angeles, CA.
• Parking: Onsite parking is first come, first served, with street parking available.
